Form is a new IRS-sanctioned form issued by the Public General Health Care Fund for providing taxpayer information to Health Insurance Marketplace (HIPAA) and Medicare Advantage Enrollment Agencies (Makes) on certain enrolled income-related conditions. The purpose of Form is to provide taxpayers with information on their income-related coverage conditions under the Marketplace and Makes; to determine whether those enrolled are entitled to certain types of coverage; and, if so, to inform the taxpayer of how he or she can request a notice of claim adjustment (NAC) and an application for Medicaid if coverage provided to the enrolled (and for which the enrolled was an exempt individual) is not consistent with his or her income. To ensure effective data matching of the income information provided to the IRS to the Makes under Form, the IRS requires issuers to upload their data and that of other issuers on the PGC Healthcare Services (HSS) data processing system and to complete the HIPAA Individual Exchange Database (IED) by March 30, 2012.
